The client is a U.S.-based multi-specialty outpatient provider group delivering primary and specialty care across multiple clinic locations. With a high volume of scheduled appointments, no-shows had become a persistent operational and financial challenge.

Connected scheduling architecture for appointment recovery

The scheduling module was deployed on Microsoft Azure using React, .NET Core, Azure SQL, Azure API Management, Twilio, FHIR R4, HL7 v2.x, SMART on FHIR, OAuth 2.0, RBAC, and SSO.

The architecture connected: Appointment Created → Reminder Engine → Patient Confirmation/Rescheduling → Cancellation → Waitlist Matching → Patient Outreach → Slot Recovered.

Twilio supported automated SMS, email, and voice communication, while the EHR remained connected to the underlying patient and appointment records. Azure API Management provided the integration layer for connected services.

Security controls included SSO, RBAC, OAuth 2.0 authentication, HIPAA-compliant audit logging, and encrypted data storage and transmission.
